Living with mental health

How we can help

When the outreach service receives a referral, we will contact you to discuss your current situation and advise appropriately according to your needs.

As well as signpost and refer to external agencies we can also refer to our internal services; Health & Wellbeing team, Money Advisers, Job coaches and the Domestic Abuse team.

Helping to build self-confidence

If after giving you advice, information and guidance you require additional support, the Outreach Service may offer a 6 to 8-week short term intervention support.

The aim of this support is to build self-confidence and promote a sense of empowerment. During this time the Outreach Worker can work with you to establish new ways of managing your situation, techniques to help you cope day to day, as well as assist you to navigate the NHS & Adult Social Care system, co-ordinating the appropriate external support as required.
